What are your opening hours?

The Ormuz Avenue Family Practice is open from Monday to Friday 8.30am-5pm.

How do I make an appointment?

Consultations are by appointment. If your Doctor is not available at the time you wish to be seen, then our Receptionist will assist you in making an appointment with the first available Doctor. Your previous Health Records are accessible to all the Doctors within the practice.

The aim of this practice is to keep appointments running to schedule. Priority must however be given to urgent cases when necessary, and this may cause delays.

Our Reception staff will endeavour to inform you of any delay when you arrive. If you require a longer appointment please advise our Receptionist at the time you make your appointment.

What happens on my first visit?

When you first visit the Ormuz Avenue Family Practice, you will be asked to complete a New Patient Questionnaire to provide your practitioner with background information about your past and present health.

During your consultation, your practitioner will review your past and present health with a view to establishing a diagnosis for any current health needs and developing your treatment plan and to plan reminders to monitor your future health needs. Depending on your individual needs you may require further investigations such as imaging or pathology tests, further visits or consultation by another member of the practitioner team.

Our practitioners will discuss with you the best way to treat your health condition. We have extensive knowledge of both public health and private health referral pathways and can give you sound advice when referral to a specialist practitioner is needed.

What length of appointment should I ask for?

Standard consultation             up to 10 minutes
One ailment (eg cold or new medical problem); or
Referral to specialist for a new condition; or
Review of a chronic medical problem; or
Skin Cancer checks.

Long consultation                    over 20 minutes
More than one ailment; or
First time visit for someone with multiple health issues; or
Pap smears; or
Well person checks; or
Life crisis; or
Childhood vaccinations; or
Travel vaccination; or
Centrelink forms to be completed.

Extended consultation             over 30 minutes
Insurance medicals; or
Pre-employment medicals

Our Fees

Bulk-billing applies for:

Children under 16 years old
Health Care Card holders
Senior Health Card holders
DVA Gold card holders
DVA White card holders (for entitled conditions )
All procedures performed within the practice
eg. ECG, Spirometry, Skin biopsies and Skin Surgery
Chronic Disease Care Plans (GPMPs and TCAs)
Medicare funded comprehensive Health Assessments


Private Fees apply for:

All Home Visits and consultations at the practice for patients not covered under Medicare guidelines for Bulk-billing.

A ten dollar administration fee is added to any accounts that are not settled on the day of consultation.
All fees include GST where applicable.
EFTPOS Debit Card and Credit Card facilities are available at our reception.

Our Receptionist can process your Medicare Claim for you electronically at the time of payment. Your Medicare rebate will be deposited into your bank account the same day. Alternatively, you may claim your Medicare rebate by taking your Receipt/Medicare Claim form personally to the local Medicare Office which is located in the Stockland Shopping Centre (at the corner of Bowman Road and Bulcock Street) just inside the entrance from the First Avenue car park.

How do I collect my medical test results?

Medical test results usually require a consultation to discuss the interpretation and significance of the findings. Please don’t assume that you will be called with the results.

Can I contact my practitioner by phone?

As we cannot guarantee that practitioners will be available at the moment you call, our receptionists will take a message and pass that on to the relevant practitioner at the first opportunity. However, in general you will be asked to make an appointment to discuss your health with your practitioner.

Do you make Home Visits?

Patients who are too incapacitated or unwell to be brought to the practice may request a home visit.

As home visits are usually attended to at the end of a consulting session, please call as early as possible.  Where possible we encourage patients to attend the surgery, as it is more effective and efficient to conduct a consultation where all facilities are available.

A private fee is charged for this service.

 What if I need to cancel my appointment?

If you are not able to keep your intended appointment, please phone our Receptionist as early as possible so that the appointment time can be offered to another patient and so that we can arrange another time that is more suitable for your needs.

A cancellation fee of $20.00 may be charged if you fail to attend your appointment without prior notification.

How do I access After-Hours Health Care?

Ormuz Avenue Family Practice provides 24 hour care for our patients in conjunction with National Home Doctor Service.

National Home Doctor Service provides after-hours home-visit services from 4pm weekdays, 10am Saturdays, all day Sunday and public holiday and they will write to inform us of their visit to you. Concession Card holders of this practice are bulk-billed, otherwise Private fees apply.

Is my medical record kept confidential?

Yes, your medical record is treated with the upmost respect and confidentiality. We have a centralised record system which means all practitioners keep their information in one place. This makes it easier to coordinate your care.

Your information will not be released to anyone without your prior written consent.
